New Patient Registration Packet
You will receive a communication from our office with a link to complete all new patient registration information prior to your appointment. Should you have any questions upon receipt please do not hesitate to give us a call. It helps us a great deal if you are able to complete these items in advance. If unable to do so, please arrive at least 15 minutes early for your appointment. We appreciate your assistance in making sure we are able to proceed with your child's appointment at the scheduled time.
Established Patient Update
We do seek to make sure we have up to date information on all of our patients. If anything has changed since your last visit, please be sure to let us know. We will send a request for updated information and it truly makes a difference for us to be aware of any changes since your child's last visit.
Requests for Patient Information
If you would like to reqeust access to your child's Personal Health Information, or would like your child's records released to a new provider we are ready to help. Just give us a call and we will send you the appropriate electronic forms.
At times families find it necessary for someone other than a parent to bring their child to an appointment. If this is the case, please let us know so we can obtain written authorization in advance.
Service Animals
Coastal Maine Pediatric Dentistry recognizes the importance of service animals and welcomes them in our practice. Emotional support animals, comfort animals, and therapy dogs are not service animals under Title II and III of the Americans with Disabilities Act. Please, keep both pets and patients safe by leaving non-service animals at home when visiting medical facilities. Thank you for your cooperation.
